The nation's top safety investigators concluded there were multiple systemic failures that led to a midair blowout during the flight of an Alaska Airlines Boeing 737 Max jet last year.

Boeing failed to provide adequate training, guidance and oversight to prevent the midair cabin panel blowout in a January 2024 flight, the National Transportation Safety Board said.

The nation's top safety investigators concluded that a lack of basic safety processes at Boeing, coupled with an inexperienced workforce, contributed to the door plug blowout in January 2024.
